The Health Information Management (HIM) Department at Citizens Medical Center maintains and protects all patient medical records while ensuring compliance with privacy regulations. We are committed to helping you access your health information in a secure and timely manner.

Who Can Request Records?
Patients
You have the right to request copies of your own medical records at any time.
Parents/Guardians
Parents or legal guardians may request records for minor children under their care.
Legal Representatives
Representatives with legal guardianship or Durable Power of Attorney may request records with proper documentation.

Pick up your records in person with photo ID, or have them mailed to your home address.
What You Need to Know
Privacy & Security
- Photo ID required for in-person pickup
- Records cannot be faxed or emailed to personal addresses for confidentiality
- Representatives must be listed on authorization form with photo ID
Fees & Delivery
- Fees may apply based on number of pages and format requested
- You will be notified of charges before records are sent
