We also believe that care is most effective when patients, families, and healthcare providers work together as partners. As a Patient, You Have the Right To:

Access & Treatment

You have the right to a reasonable response to your request for care and treatment, regardless of race, creed, sex, national origin, disability, or payment source.

Dignity & Respect

You have the right to considerate, respectful care in a safe, secure setting, free from all forms of abuse or harassment, with privacy protections for personal information.

Informed Decision-Making

You have the right to request or refuse treatment and to remain informed about care outcomes. You retain the right to provide written consent for medical procedures except in emergencies.

Disclosure of Outcomes

Healthcare providers must inform you about unanticipated outcomes of care, treatment, and services related to adverse events.

Care Planning

You have the right to participate in the development and implementation of your treatment plan and to formulate advance directives delegating decision-making authority.

Communication Support

You have the right to access interpreters for language barriers and information about your healthcare provider credentials.

Privacy & Confidentiality

You have the right to have your medical information kept confidential and to review your medical records in accordance with HIPAA regulations.

Grievance Rights

You have the right to submit a written or verbal grievance about the care, services, or staff without fear of reprisal.

Your Responsibilities

As a Patient, You Are Expected To:

Provide Accurate Information

Supply complete and accurate medical history, including current medications, allergies, and past treatments.

Understand Your Care

Ask questions if you do not understand your treatment plan and confirm your understanding of proposed procedures.

Follow Treatment Plans

Follow the recommended care protocols and treatment plans agreed upon with your healthcare providers.

Meet Financial Obligations

Provide insurance information and settle financial obligations promptly. Ask about financial assistance if needed.

Respect Hospital Policies

Follow hospital rules and regulations, including visiting hours, safety protocols, and the smoke-free campus policy.

Respect Others

Respect the rights, privacy, and property of other patients, visitors, and hospital staff.

Filing a Grievance

If you have concerns about the care or services you received at Citizens Medical Center, we encourage you to speak with us. Your feedback helps us improve our services and ensure quality care for all patients.

You may submit a written or verbal grievance at any time, and we will address your concerns promptly and respectfully. Speaking up will not affect your current or future care at our facility.

How to File a Grievance:

Speak With Your Care Team

Talk to your nurse, doctor, or department manager about your concerns.

Contact Patient Relations

Call (361) 573-9181 and ask for Patient Relations or Administration.

Submit in Writing

Write to the Patient Relations Department at 2701 Hospital Drive, Victoria, TX 77901.

Advance Directives

You have the right to formulate advance directives that express your wishes about medical treatment should you become unable to make decisions for yourself. These may include a Living Will, Medical Power of Attorney, or Do Not Resuscitate (DNR) orders.

We encourage you to discuss advance directives with your family and healthcare providers. Our staff can provide you with information and forms to help you document your wishes.
